Summary of Training or CME : 

Activity Description
As a participant in the UW SMPH CS2day Performance Improvement activity, you will commit to complete online documentation, review your data, and create a plan for change and improvement. Additional time will be spent implementing changes to your practice over the course of 3 months, and re-evaluating upon completion. In total, it should take between 3-6 months, but this amount of time will vary slightly with each individual.

Goals (Purpose) of Program
The goal of this activity is to help providers improve treatment and management of their tobacco using patients in order to increase tobacco quit rates.

Learning Objectives
Participants of the CS2day PI CME activity will be a part of a national movement to help patients quit smoking through self-assessment and practice improvement where they will:

1.       Identify areas for improvement in knowledge, attitudes, skills, and performance relating to treating tobacco use and dependence
2.       Create an action plan to improve the care of patients with tobacco use and dependence
3.       Utilize tobacco cessation performance measures to analyze practice performance
4.       Implement personal and system changes to improve the care of patients with tobacco use and dependence

Target Audience
Physicians, nurse practitioners, physician assistants, and other healthcare professionals who treat patients with tobacco use and dependence and want to help them quit through performance improvements should participate in this CME activity.
